Student-Initiated Forms

- Diploma Reprint Request Form 鈥 Request another copy of your diploma. If you did not receive your original diploma, please contact the Registrar鈥檚 Office at registrar@fresno.edu.

- FERPA Release 鈥 Grant a person permission to review your academic records. To submit a FERPA release, go to , click on the user icon in the top right corner and select 鈥淪tudent Records Release鈥 from the drop-down menu.

You can report a change to your phone number or address in . On the left, click "User Options," then "User Profile," and insert correct information. If you are not a current student, please email the Registrar鈥檚 Office at registrar@fresno.edu.
- 鈥 Register for directed studies, independent studies, a graduate version of an undergraduate course or special units.
- 鈥 Appeal the final grade you received in a class. Submit this form only if you have talked with your instructor first and cannot come to a resolution. View the full policy.
- 鈥 Students using Department of Veteran Affairs-issued benefits, such as Post 9/11 GI Bill, will complete this form each semester to confirm the number of enrolled units to be submitted to the VA.
- 鈥 Withdraw from all classes and leave the University.

Program Director Initiated Forms
- 鈥 Use to request the creation of a new course in the catalog and system.
- 鈥 Use to change characteristics of an existing course (e.g. title, prerequisites, course description, etc.)

- 鈥 Program Directors use to request and update to the current program.
Instructor Initiated Forms
- 鈥 Use when a clerical or computation error has been made, or for updating a grade due to incomplete work.
Advisor Initiated Forms
